McCarty Cancer Foundation

McCarty Cancer Foundation was created by NHL star, Darren McCarty formerly of the Detroit Red Wings now with Calagary Flames. With Darren's relentless efforts and those of many devoted individuals, the McCarty Cancer Foundation is dedicated to raising awareness of multiple myeloma and improving the quality of life of myeloma cancer patients, while working toward prevention and a cure.

Third Party Event Application Terms and Conditions
1. Any use of the McCarty Cancer Foundation name on any promotional material must be approved by the MCF prior to printing and public distribution. Promotional materials should indicate that the event is SPONSORED BY (NAME) TO BENEFIT THE MCF. AT NO TIME SHOULD ANY MATERIALS INDICATE THE EVENT IS MCF SPONSORED ACTIVITY.
2. The Sponsoring Organization assumes full responsibility for conducting the event without participation or support from the MCF or its representatives.
3. Since the MCF is to receive proceeds from the event, the portion that the MCF is to receive must be clearly stated on all materials and communications to all participants. Checks should be made payable to your organization or designated account.
4. The Sponsoring Organization , Individual, or Club is responsible for all expenses and the payment of such incurred by this event. Further, the MCF can not process checks made payable to the foundation and reimburse Sponsoring Organization, Individual, or Club for event expenses.
5. The Sponsoring Organization, Individual, or Club is responsible for securing and paying all licenses, permits, or insurance the event may require.
6. The Internal Revenue Service has specific guidelines regarding the deductibility of contributions to the charitable organization. Promotional material must state the fair market value of anything the participant is receiving as part of the participation fee.
7. The Sponsoring Organization, Individual, or Club is not authorized to use the MCF’s charitable solicitation license number nor its tax-exempt number.
8. The Sponsoring Organization, Individual, or Club agrees to assume all risks of damages or injuries, including death, to property or person used or employed on or in connection with the event, and all risks of damages or injuries, including death, to any persons or property wherever located, resulting from any action, omission, or operation, whether such action, omission, or operation is attributable to the Sponsoring Organization, Individual, or Club, any of its subcontractors, and any other person. Sponsoring Organization, Individual, or Club shall indemnify, hold harmless, and defend the McCarty Cancer Foundation, their employees, agents, volunteers, servants, and representatives, from and against any and all claims and demands of whatever nature, regardless of the merit thereof, which may be asserted against the McCarty Cancer Foundation or on account of any such damages or injuries, including death, whether or not such damages or injuries, including death, are caused in part by the negligence of the McCarty Cancer Foundation, their contractors, employees, agents, servants, or representatives.

9. INSURANCE REQUIREMENTS—The Sponsoring Organization or Individual must provide, prior to approval of the event, a certificate of insurance naming the McCarty Cancer Foundation, its volunteers, officers, and directors as an additional insured. This certificate must list insurance companies rated “A” by A.M. Best, or better. This carrier must be licensed and admitted in the State of Michigan. The certificate must show Comprehensive General Liability with minimum limits of $1,000,000. If the Sponsoring Organization is required by the State of Michigan to carry Workers Compensation, this certificate will exhibit the same. Based on the type of event, our insurance consultant may ask for (automobile coverage, liquor liability coverage…etc.) additional coverage’s as needed. Please fax your insurance certificate to (248) 258-1964 for approval. Please be advised the McCarty Cancer Foundation can only approve an event after this certificate is approved.
10. In the event the application is approved, the money must be submitted to the McCarty Cancer Foundation no later than one month after the completion of the event. Follow up phone calls will be made to check the status of event.
